Prefer to ship or request a pickup? Summit-Horizon makes it simple. Send conventional impressions, stone models, and bite records with a completed Rx. St. Louis metro practices can schedule complimentary pickup. Practices outside the metro can ship cases from anywhere in the U.S.

TRADITIONAL CASE SUBMISSIONS
Prefer impressions or models? We make that just as easy.
Fill Out RX Form (Fixed or Removable):
- Add patient ID
- Tooth numbers
- Shade info
- Notes
Package Securely: Include the RX with your impressions/models.
Send Your Case
- Local Pick-Up: Call us to schedule a driver.
- Shipping: Use your preferred carrier to ship directly to our St. Louis lab.
Case Confirmation: We’ll review and confirm receipt, so you know your case is in progress.
